Identifying & Resolving Drug Therapy Problems Across the Continuum of Care

This is an online enduring/home-study CPE module. This program will take approximately 6 hours to complete. This continuing education activity is supported by a cooperative agreement between the University of Hawaiʻi at Hilo and the CMS Innovation Center. The target audience is pharmacists working outside of dispensing roles in hospital and community settings.

Learning Objectives

Upon completion of this activity, participants should be better able to:

  • Identify patients at risk of medication-related hospitalization and emergency/department use
  • List the clinical goals for each medication for each patient at risk
  • Identify drug therapy problems for each patient at risk
  • Implement solutions to resolve drug therapy problems

    • Minimum memory: Minimum 512 MB; Recommended: 2 GB or more
    • Processor speed:
      • PC — Pentium IV, 3.0 Ghz, equivalent or better. Recommended: Dual Core processor or better.
      • Mac — PowerPC G4 or Intel Dual Core Processor (or better)
    • Multimedia components required by the learner: Speakers
  • Web Browser:
    • Mac – Safari 6 or later is recommended
    • PC — Chrome 51 or later is recommended
    • Required Browser Plug-ins:
      • Adobe Acrobat Reader for PDF files
    • Adobe Flash Player for Adobe Flash content.
    • Javascript and cookies must be enabled.
    • Pop-up blockers must be disabled.

  • Internet Speed (recommended):
    • Broadband (DSL, cable, etc.). Dial up service is not recommended.
    • When tested from a 10 Mbps connection, the module required approximately 2 minutes to load. Lower connection speeds will take significantly longer.

ACPE The University of Hawaiʻi at Hilo Daniel K. Inouye College of Pharmacy is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This is an application-based continuing education activity of the University of Hawaiʻi at Hilo, an equal access/equal opportunity university (0516-0000-22-025-H01-P through 6/6/2025).

To receive the 6.0 contact hours of continuing education credit, pharmacists must complete the entire program, including successful completion of all learning assessments that document target competencies, the program evaluation, and program request for credit. Partial credit is not offered for CPE Activities.
