Required Items to be Submitted by June 1, 2017

  1. After all courses are completed, official transcripts from each institution attended should be addressed to:

    Students do not need to send transcripts from institutions within the University of Hawai‘i system, which includes UH Manoa, UH Hilo, UH West Oahu, Maui College, and the community colleges.

    Verification of all complete pre-requisite courses:

    • All pre-requisites must be successfully completed with a grade of C or better prior to the first day of orientation on August 14, 2017. (Please Note: All students must be on campus for orientation week from 8/14/17- 8/17/17. Dates subject to change.)
    • If coursework is not fully completed by June 1, 2017 the student must submit a letter from the instructor verifying that prerequisite coursework is in process and that a satisfactory grade is being earned.
    • If course work is completed after June 1, 2017 an additional updated official transcript must be sent to address above once your grade has been posted and prior to the first day of orientation.
    • Please Note: The transcripts sent to PharmCAS during the application process will not fulfill this requirement.

    • Proof of Health Insurance:

      All students must maintain current health insurance at all times while enrolled in the Pharm.D. program. You must provide the Office of Experientials with a copy of your current insurance card and submit additional cards to their office if changes are made to your policy.

      Health insurance for UH students is available through the HMSA UH Chaminade Student Plan. In addition, students have the option to obtain health insurance that is not associated with the University.

      All students must provide proof of health insurance starting fall semester of 2017.

Prepare for Your Arrival

  1. Get your UH username and access your student account (MyUH):

    Get a UH Username and access your student account through MyUH. (Please Note: You will need your UH Number (xxxx-xxxx), which will be emailed to you upon matriculation. If you have not received your UH Number, please call us at (808) 932-7140.) Learn more about managing your UH Username.

    Class of 2021 on Facebook Class of 2020 on Facebook is a great place to interact with other matriculated students, look for shared housing, and discuss issues related to your up-coming arrival.
  3. Review the welcome letter and important information

    More information about registration, laptop requirements, parking, attire, etc. will be coming soon.
  4. Mark your calendar and attend orientation:

    Orientation is mandatory and will be held August 14 - 17, 2017 at 722 South A‘ohoku Street. (Please plan to arrive at orientation at 8:00am on August 14, 2017. Dates subject to change.)

    Mailing address, email address, and phone changes should be reported immediately by logging into the MyUH Portal and clicking on View/Update Address, Phone. Please submit a student personal data update form for name and permanent address changes. Evidence of the legal name change is required.
